Hi,
Actually in default,Those informations are coming by tcalling some built-in templates.
EG: external_layout,external_layout_standard..etc
In the reporting templates,When we tcall those templates we'll get those informations on the headere and footer.
If you customized the template,Make sure you're included this thing in it.Otherwise you'll have to create custom template for it too.
