How to create Custom Checkbox field on Setting page in Custom Module

Question

Hi there,
We have created a simple custom module, which has normal tree view & form view of a custom model.
We want to have a Configuration page under settings, where we'll have 3 checkbox fields.
Based on the state of these checkbox fields, we want to display some additional data on form view.
Any idea, how to create and enable save feature in checkbox fields on setting page?
