in CRM leads, when we click convert to opportunity button then that lead is convert to opportunity.

similarly I am trying to create a "potential(new stage)" stage between opportunity and quotations.

inside the opportunity i placed a button convert to potential,when I click this is button,showing "form view could not be loaded error".

can anyone help why I am getting this error.
