Inherit view - Surveys

Question

Can someone explain to me as if a was a child how to inherit a view step by step with some screenshots.
After that, I also need to change the layout of a survey so I can add a signature or an image, or just change de blue, purple, or gold color. Any ideas? I´ve search in other post but I do not understand.
