For using any custom module in the server.You have to download and copy the required custom modules to the server then give the path of the folder of custom modules into the .conf file of Odoo and just restart the service of Odoo

I would not recommend installing Odoo 11.0 and newer on an old Ubuntu version like 16.04 and also not on a non-LTS Ubuntu version like 19.04. Just my 2 cents.
You can use an unmanaged server provider if you can manage the server yourself (ex. Vultr, Linode, DigitalOcean) and if you can't manage the server yourself you can use a managed provider like SolaDrive. If going with the latter option they will likely migrate it for you, but you'll have to ask to make sure.
