The easiest thing that you can do from your side is, create a question in quiz manually from the user interface, then using odoo export option, you can export it with needed fields, make sure to export the required fields(which is underlines/bold in export wizard)

You can export it in either excel or csv format, once the export is done, you can use this downloaded file as a template for further imports by adjusting the values.
