Hi this is a small question but i dont know how to do it, my concept is i've 5 fields i want to add the heading of the fields, i want like this screenshot.

have you tried both way?
In that link they told change in .CSS file but i dont know how to change that.
You need to apply changes shown in 2nd link in base.css file (path : addons/web/static/src/css).
It went wrong, other option how to add Percentage symbol in the end of textbox
