I seem to have had a very similar issue. The tools tips don't make it clear on what to do and/or in what order. Sometimes I got the Conditional  Question, but not the Answer question. What seemed to work for me was:

1. to make all the questions first with their answers

2. save them and save the survey itself.

3. Then go back into the conditional question and try to see if it allows you to selected the question and answer to trigger it.
