I get last log when install or upgrade module

I also had the same problem as you. I fixed it by going to the XML menu file, finding the line: web_icon= and changing the comma to '/' .
For example: web_icon="account,static/description/icon.png" change to web_icon="account/static/description/icon.png" Then the update process was successful. This solution only work for window. If you use linux just keep using the comma
Maybe your module 'multi_level_approval' is error somewhere, you can post the log or ask the author to help
