How to create Outpass slip module in odoo11 community version..

Question

Outpass means if an employee for any urgent reason want to leave then he should  fill the outpass form it contains the the name of employee,total duration of time going out,time going out,time going in,reason for going out, department of employee..Outpass slip should be approved or refused by HR.Then only he/she can take...
How to create this outpass module...I have attached an example of slip over here...
please help...

Hi,
You can create a new model for the employee to request the out pass and also along with the same you can add a qweb report to print the out pass.

The link is of v10, but a slight change will work in v11.
