For online exams there is a module available : https://www.odoo.com/apps/modules/10.0/exam_test_quiz/

Survey by default available in the Odoo. 

If you are looking for a software to manage school or educational institution have a look at open educat modules available in the Odoo apps : https://www.odoo.com/apps/modules/10.0/openeducat_erp/
. May be having doing some changes in it will give you want you need.
