As there aren't any templates available yet in Odoo, after some trial and error I managed to get the attached Bank Import template working.
The trick was to reduce the amount of columns.
I hope it's useful to others.

Don't worry about this. There's no upload or delete capability in Odoo. We will probably stop attempting to use the software soon. Life is a bit short for this software.
