The module you buy from the Appstore is a one time fee. Once you buy it, it is yours. you can download it multiple times if there is any update / bug fix in the module.
Once you buy a module of specific version and in future if the module is available in another version, you can download the module of latest version as well without paying anything.

The module(s) you buy from the appstore i.e., custom modules are billed just once i.e., one time settlement and the module(s) that you buy from Odoo e.g. modules from enterprise version involves a subscription type payment which is billed yearly.
In addition to below answers, keep eye on the license given for the module, to know whether the redistribution or modifying of the code is allowed or not.
