Hello

I have a checklist for purchase product. For now I have added them static but it's long list I want to add them dynamically. 

There is there field 

1: Category

2: Checklist

3: Option (Yes, No, Don't know) 

I have many category if some one create category and add 100 or more checklist associate to specific  category it show under that category and automatically append that checklist under selected category while creating checklist and so on 

Please let me know how can I can achieve that.
