How to add menu item action in CRM module

Hi Team - i am trying to add a menu action in existing module CRM but don't know how to do it. I watched video https://www.youtube.com/watch?v=Ap-op_oEDXY but its not useful. It only talks on menu action whereas i cannot see any menu action in CRM. Can someone specifically guide me.
