I experienced such exceptions , I wanted to know where they are set up or coming from? And how it determines that the activity should be assigned to which user ?
In my case it automatically picked the assignee which was already archived and the assignee is not a follower of any related document.
There is no automated action set up in my case which causing this.

Actually the above screenshot, 'Stretch film' was initially added in the purchase order. Then the same line item was edited and updated with another product. Now I see the exception messages under receipts.
The user who created the changed item master is receiving mail alerts about the exception. Master creator will be added as a follower by default of the product master.

In my case my user make the qty zero but the activity is assigning to someone else's user.
In your case it is also mentioning tha change in qty from 20 to 1 but no clue of change in product I can see in screenshot.
Please go and check, who is created the product .
