What configuration error will create a duplication of the journal to account for the Work Centre costs when completing a Manufacturing order?

The MO overview shows a breakdown of the Work Centre  costs and associated labour cost, in accounting there is two journals posted to account for the sum of these costs. 

Also the value of the journals are not the same as the values on the MO overview, about a 1% variance on the Work Centre costs as well as the output stock values
