The email designer is based on website builder where we can edit the HTML of a page, but there does not seem to be a way to alter the contents of an email template, just drag and drop from website builder toolbox. It's very usefull to add image alt attributes, NOFOLLOW links or any other HTML custom code. How can this be done?

To edit the HTML of your website, you must be logged in with permissions to edit your website then from the website's public front-end you click Customize then HTML Editor located on the top right corner of the website.
How do I use HTML editor for email templates if I do not have a public odoo website? (Just using CRM and market campaign modules)
