Many months ago I modified the /contactus page through the web WYSIWYG. Unfortunately not realizing I messed up the contact us form.

Is there a way I can restore the original stock page to working order again? I moved my modified version to /contact-us2

But I'm having trouble figuring what to do next.

Appreciate some guidance.
