"Odoo Error" is not a question. I have edited your post to make the title a question. Hi,

Are you trying to define new form or trying to inherit an existing form and making changes ? 

If you need to add a new form you can add it as follows:

    <field name="name">hospital.patient.form</field>
    <field name="model">hospital.patient</field>
    <field name="arch" type="xml">
        <form>
            <sheet>
                <group>
                    <group>
                        <field name="name"/>
                        <field name="age"/>

                        <field name="note"/>
                    </group>
                </group>
            </sheet>
        </form>
    </field>
</record>

Or if you are looking to inherit an existing view, you can do it like this:

    <field name="name">sale.order.inherited</field>
    <field name="model">sale.order</field>
    <field name="inherit_id" ref="sale.view_order_form"/>
    <field name="arch" type="xml">
