I attach three images at the bottom. What I want is fields layout as the first image when "Control = One" and "Control = Two". Code are given below. Could someone please point out where I should revise to achieve the desirable fields' layout in UI. Thank you a lot.

Control = One. Incorrect fields' layout where 'Type and Team' are offset.
Control = Two. Incorrect fields' layout.
