I remember having the same problem long ago and the reason often was weird and dramatic, resulting in several important modules uninstalled and data loss. I don't remember exactly the reason but it was something like upgrading to a wrong or lower version, or uninstalling some module with broken dependencies that uninstalls some base modules, or triggerring a wrong upgrade on the wrong database because of a persistent session pointing to another db.
Anyway if your menus are in total mess in means you probably lost the toplevel menus, which are defined in the base modules. Try to look at the state of the base modules directly in the database (something like: select name, state from ir_module_module)

In order to update modules, I ran my server with the flag --update=all and it resulted in my menu bar including all "second level" menus. For instance, we have "Project" on the top bar, but also its sub-menus "Tasks", "Task Stages" and so on.

This is a total mess and makes it impossible to work on this instance.

Any help about how to address the problem would be great. Thanks a lot.
