when the user start the survey the survey prefilled with e-mail, the name of the user and the phone number from the system depend on the user info exist

To do that, you will need to use "Share and Invite by Email", from survey form view, click on "Share and Invite by Email" button and share it with existing contacts and select the contact.
thanks walid for your interest, but you misunderstand me, i need when the user open the survey some fields automatically filled based on the user info
