For the USA, is there a module that adds a list of all of the US Counties, Parishes, Borough's etc?

Try https://www.openerp.com/apps/7.0/l10n_us_administrative/

The module adds all official United States political and geographic regions - states, territories, counties/city-counties, parishes (Louisiana) and boroughs (Alaska).

When entering an address, once a valid state is chosen, the county equivalent is filtered.

A new menu at Sales --> Configuration --> Address Book --> Localization --> USA Counties is added to maintain this data.
