            <field name="model">ir.sequence</field>

            <field name="arch" type="xml">

                <form string="Sequences">

                    <group col="4">

                        <field name="name"/>

                        <field name="code"/>

                        <field name="company_id" groups="base.group_multi_company"/>

                        <field name="active" groups="base.group_no_one"/>

                    <page string="Sequence">

                        <group col="4">

                            <field name="prefix"/>

                            <field name="suffix"/>

                            <field name="padding"/>

                            <field name="number_increment"/>

                            <field name="number_next_actual"/>

                            <field name="implementation"/>

                        <group col="3" string="Legend (for prefix, suffix)">

                                <label colspan="2" string="Current Year with Century: %%(year)s"/>

                                <label colspan="2" string="Current Year without Century: %%(y)s"/>

                                <label colspan="2" string="Month: %%(month)s"/>

For the label, you can simply inherit the view and then add the label.  But adding the label would not make 'kode cabang' available to be used for sequence numbering you need to inherit modify ir.sequence some of its methods to suit your needs.
